Opens the single direct PDF/VCR template branch and declares every field that replacement placeholders may reference

Remarks
The method writes a direct Sect child of StructTreeRoot with /O /GTS_Template, a /GTS_Fields PDF-name array and optional /GTS_Pages
Only one template root is allowed and it must be opened with no other structure element active
Close every placeholder before calling EndPDFVCRTemplate
