Associates one page-local MCID with an existing structure element through an MCR dictionary and the page ParentTree

`PageIndex`	Zero-based page index that owns the marked content
`MCID`	Non-negative marked-content identifier that is unique on the page

The method appends <</Type /MCR /Pg ... /MCID ...>> to the structure element K array
The page receives or reuses its StructParents key
Invalid pages, negative MCIDs, nil elements, and non-array K entries are rejected
