Code identifies the loss and Description explains the affected behaviour; available codes include hidden-annotation-removed, link-interaction-removed, appearance-synthesized, annotation-interaction-removed, form-fields-removed, signatures-invalidated, embedded-files-removed, information-dictionary-normalized and presentation-metadata-removed

Page and annotation indices are zero-based positions in the working source snapshot before removal; negative indices indicate a document-level change or an unavailable source location

ObjectNumber identifies the original affected annotation object when available and is negative for document-level changes

Appearance synthesis and interaction removal can produce separate entries for the same annotation, making the visible-content decision and the lost interactive behaviour independently reviewable
