Indexフォームフィールドのインデックス
ChoiceType1 = フォームフィールドをスクロール可能なリストボックスに設定 2 = フォームフィールドをドロップダウン式コンボボックスに設定 3 = フォームフィールドを複数選択できるスクロール可能なリストボックスに設定 4 = フォームフィールドを編集ボックス付きのドロップダウン式コンボボックスに設定
