Index	Indice du champ parent. Titre du nouveau champ de formulaire. Le titre ne peut pas contenir le caractère point "."

FieldType	Type de champ à créer : 1 = Texte 2 = Bouton-poussoir 3 = Case à cocher 4 = Bouton radio 5 = Choix 6 = Signature 7 = Parent

0	Le nouveau champ de formulaire n’a pas pu être créé
Non-zero	Le champ de formulaire a été créé avec succès, et ceci est l'index du nouveau champ
